My Tears, My Rewards® Program Information

How does the RESTASIS® My Tears, My Rewards® Program work?

Once you have sent your original RESTASIS® pharmacy receipt to us, you will receive your personal organizer. In Tab 2, you will find your preprinted certificate self-mailers you can easily tear out. After you fill each of your prescriptions for RESTASIS® Ophthalmic Emulsion, enclose the pharmacy receipt in these self-mailers and send these back to earn your rewards.

When you have sent in your second certificate self-mailer and 2 pharmacy receipts, we will mail you a rebate for up to $20 that can be applied to your next RESTASIS® prescription.

When you have sent in your fourth certificate self-mailer and 4 pharmacy receipts, we will mail you a rebate for up to $20 that can be applied to your next RESTASIS® prescription.

When you have sent in your sixth certificate self-mailer and 6 pharmacy receipts, you will receive a free magazine subscription or reward of your choice.

When you have sent in your seventh certificate self-mailer and 7 pharmacy receipts, we will send you a voucher for a free 30-day supply (2 trays—60 vials total) of RESTASIS®.


My doctor prescribes a 3-month (90-day supply) of RESTASIS®. How does the RESTASIS® My Tears, My Rewards® Program work for me?

For enrollees with a 3-month (90-day supply) prescription refill:

  1. Mail in your first preprinted certificate self-mailer along with your original 3-month (90-day supply) mail order receipt (proof of purchase) to

  2. Mail the receipt for your second 3-month (90-day) supply using the self-mailer for month #4 (skip the self-mailers for months 2 and 3).

  3. Mail the receipt for your third 3-month (90-day) supply using the self-mailer for month #7 (skip the self-mailers for months 5 and 6).

We'll credit your 3-month receipt toward your earned rewards for that 3-month period. If you redeem a receipt for your 3-month (90-day) supply for month 6 or 7, your program status will carry over to the next 7-month rewards sequence—as long as you continue to use RESTASIS®.


After I fill my sixth RESTASIS® prescription, what are the reward choices I can choose from?

At month 6, you may choose one of the following rewards, which are subject to change without notice:

  • A one-year subscription to Prevention® magazine
  • A $25 dining gift card
  • A $25 Barnes & Noble® gift card
  • A $25 Fandango® movie card, good for two tickets

When will I get my magazine/gift card?

Allow 4 to 5 weeks for delivery of the magazine. If you chose to receive a gift card, allow 6 to 8 weeks for delivery.

Where do I send my receipts and certificate self-mailers?

Put your receipts in the certificate self-mailers inside the organizer and send them to the following address (which is also printed on each self-mailer):

After I receive my voucher for a free 30-day supply (2 trays—60 vials total) of RESTASIS®, can I enroll in the program again?

After receiving a free 30-day supply (2 trays—60 vials total) of RESTASIS®, you will automatically be re-enrolled in the program, and 7 new certificate self-mailers will be sent to you.


What does Allergan, Inc. do with the information I provided in the online survey?

Your information, along with the information of other program members, is used to learn more about Chronic Dry Eye sufferers and our program members. This information will also be used to continually improve the My Tears, My Rewards® Program. Your personal information is not shared with anyone and is not disclosed to any third-party individuals or government agencies, except as may be specifically required by law.

My Tears, My Rewards® Program Timing

How much time do I have to use my rebate before it expires?

Your rebate will be valid until the date printed on the rebate.


How long before I receive my rebate?

Please allow 6 to 8 weeks for your rebate to arrive.


When will I receive my voucher for a free 30-day supply (2 trays—60 vials total)?

Your voucher will be mailed within 5 business days after your last certificate self-mailer is received. The voucher is good for a free 30-day supply (2 trays—60 vials total) of RESTASIS® Ophthalmic Emulsion.


I am concerned that I might not get all my certificate self-mailers in on time. How long will the program be available?

My Tears, My Rewards® members have 1 year from their date of program enrollment to send in all 7 certificate self-mailers and 7 pharmacy receipts. Your program expiration date is printed on the self-mailers.


I have receipts for RESTASIS® purchases prior to my enrollment date. Can I submit these receipts for credit toward my 7 purchases needed to receive a free 30-day supply (2 trays—60 vials total) of RESTASIS®?

No, only receipts dated after your enrollment date count toward your rewards.

My Tears, My Rewards® Online Account Questions

I have sent in more certificate self-mailers than my online account is showing. How frequently is the Web site updated to include my latest account information?

The Web site is updated weekly with certificate information. You should see your updated My Tears, My Rewards® account information online within 2 to 3 weeks from the date you sent in your last certificate self-mailer.


I have waited 3 weeks for the Web site to update since I last sent in my certificate self-mailer(s), and my account still hasn't been credited. Whom should I contact to resolve this issue?

Click Contact Us to send an e-mail to a Customer Service Representative. He or she will check your account and send you an e-mail within 2 business days with your updated account information.


After using the FORGOT PASSWORD link, I received a message saying I entered the wrong e-mail address or password for my online account. How can I get the correct login information or correct this problem if I think I got this message in error?

Click Contact Us to send an e-mail to a Customer Service Representative. He or she will check your account and send you an e-mail within 2 business days with your correct account login information.


I don't have regular access to the Internet. Can I check the My Tears, My Rewards® account by telephone?

Yes. You can call the RESTASIS® Patient Support Center at 1-866-572-5931 to check the status of your account, including how many certificate self-mailers you have submitted to the program.

Important Information:
RESTASIS® Ophthalmic Emulsion helps increase your eyes’ natural ability to produce tears, which may be reduced by inflammation due to Chronic Dry Eye. RESTASIS® did not increase tear production in patients using topical steroid drops or tear duct plugs.

Important Safety Information:
RESTASIS® Ophthalmic Emulsion should not be used by patients with active eye infections and has not been studied in patients with a history of herpes viral infections of the eye. RESTASIS® should not be used while wearing contact lenses. If contact lenses are worn, they should be removed prior to use.

The most common side effect is a temporary burning sensation. Other side effects include eye redness, discharge, watery eyes, eye pain, foreign body sensation, itching, stinging, and blurred vision.
